On this page you will find all the forms and notes that need to be filled out for attendance at CY and related events.
These forms can be a hassle at times but they are important for us to have so that parents can express consent for activities and know what their children are doing, what we need to look out for and how we can contact them.
If you are planning on visiting us for the first time please complete our visitors form. This gives us important information for a youth that is visiting including contact information in case of emergency.
Please also fill out our registration form if you have not done so before. The Registration Form gives permission for your child to attend CY for the current year. Please complete this forms once per year.
